DUTIES AND RESPONSIBILITIES: 

Nurse Practitioners are leaders in the mental health treatment of patients at LHS. Such duties include: 

  • Engage in direct patient contact (in person, via telemedicine, email, or phone) within the scope of practice for Nurse Practitioners, including  

  • Conduct initial health assessments including recording complete medical and psychosocial history 

  • Record symptoms 

  • Examine patients 

  • Make diagnoses 

  • Develop a treatment plan that may include medication and other therapies 

  • Maintain accurate appointment notes in the EHR 

  • Send prescriptions to pharmacies for patients  

  • Provide patient education to promote habits that will prevent diseases and maintain good health 

  • Order and interpret laboratory test results 

  • Follow up on a patient to determine the effectiveness of recommended treatments 

  • Provide health education and supportive care under the direction of the psychiatrist 

  • Collaborate with LHS medical practitioners, clinicians, and other treatment staff to support effective, multifaceted treatment planning inclusive of all services provided and facilitate referrals of patients to additional treatment opportunities to support patients’ treatment plans 

  • Advise Registered Nurses, Medical Assistants, and other medical staff regarding medical issues / medication concerns for patients in the practice 

  • Communicate effectively with primary care physicians and related patient treatment providers as needed to coordinate care including receiving/returning calls and messages from community-based providers, hospitals and related organizations 

  • Utilizes the Electronic Health Record (EHR) to review patient charts, assuring all relevant medical information is documented, thereby enhancing the medical care of patients in treatment for mental health and substance use disorders.  

  • Demonstrates understanding of medical issues and how such issues may impact mental health 

  • Respects and maintain privacy, confidentiality, and respect for patients receiving mental health and /or substance use treatment; exhibiting a supportive, unbiased and non-judgmental attitude is essential for enhancing provider – patient rapport and promotes patient compliance in treatment 

  • Consistently demonstrates professionalism, attention to detail, understanding and respect for self and others in the workplace 

  • Supports and promotes Landmark Health Systems’ commitment to providing the best treatment, including quality, care, and concern for prospective and active patients and their families 

  • Attends and actively participates in case conferences and training sessions as required 

  • Performs other duties and projects as appropriate 

    JOB QUALIFICATIONS:  

    • Graduate degree from an accredited nursing school, such as a Master of Science in psychiatric nursing 

    • Experience working in a clinical setting, 2 years required 

    • Experience working in a mental health clinical setting 

    • Experience with Electronic Health Record systems, AMD preferred 

    • Ability to communicate effectively with patients and other medical staff 

    • Strong leadership skills, self-confidence, and ability to remain calm in stressful situations 

    • Excellent analytical and critical thinking skills 

    • Exceptional active listening skills 

    LICENSURE, CERTIFICATION OR REGISTRATION: 

    • Current license to practice as a nurse practitioner in NYS 

    • Credentialed with BCBS/Highmark, United Healthcare and Independent Health
